Please see forms below and complete any that are required for your daughter. Some may need to be downloaded and completed by a physician before being returned to the School Office at Oakland Catholic. Others, like the Medical Emergency Form, may be completed online.

Self Administration of Medication Form
This form needs to be completed for students each year and returned to the School Office.
Medical Emergency Form
This form is required for all students and transfer students. Once completed, please return to the School Office.
Health Assessment Form
This form is only required for incoming 9th grade and transfer students. Once completed, please return to the School Office.
Athletics Physical Form(s)
All forms/physicals require completion by a physician dated after June 1, 2021, prior to the start of the athletics season and returned to the School Office.
